Sildenafil vs Tadalafil, and Why ED Appears on Cycle

📅 July 29, 2026 ⏱️ 2 min read ✍️ Team TPS

Sildenafil and tadalafil do the same thing on different timescales, and neither addresses desire.

How they work

Both are PDE5 inhibitors. They block the enzyme that breaks down cGMP, the signalling molecule that keeps smooth muscle in the penis relaxed and blood flowing in.

Critically, they do not initiate anything. Arousal still has to happen; these compounds make the mechanical response to it work properly. That is why they do nothing for someone whose problem is libido.

Sildenafil: short and sharp

Sildenafil acts within roughly an hour and lasts four to six. That makes it something you plan around, and a heavy meal noticeably delays absorption.

Tadalafil: the long one

Tadalafil lasts up to 36 hours, which removes the planning entirely. Food does not meaningfully affect it.

The long half-life also supports low daily use, which some prefer for spontaneity, and which has been studied for benign prostatic hyperplasia as well.

The blood pressure interaction that matters

PDE5 inhibitors and nitrates together can cause a catastrophic drop in blood pressure. This is not a mild caution. It is the reason these drugs carry an absolute contraindication.

Anyone taking nitrates for angina must not take either compound. Combining them with other blood pressure medication such as telmisartan also needs medical input rather than guesswork.

Why ED appears on cycle

Erectile difficulty during or after a cycle usually has a hormonal cause rather than a vascular one, and reaching for a PDE5 inhibitor treats the symptom while the cause continues.

The usual culprits are oestrogen too low or too high, elevated prolactin, or crashed testosterone after a cycle. All three are cheap blood tests. See aromatase inhibitors compared, cabergoline and prolactin, and PCT explained.

If the problem is desire rather than function, PT-141 addresses a different mechanism entirely.

A note worth making

New erectile difficulty can be an early marker of cardiovascular disease, because the vessels involved are small and show problems first. It is worth investigating rather than only medicating.
