Tirzepatide vs Retatrutide: Choosing Between the Two Pens

📅 July 6, 2026 ⏱️ 3 min read ✍️ Team TPS

Tirzepatide and retatrutide differ by one receptor, and that single addition is the entire decision. Both are available as SerenQ pens, and choosing between them is less about which is stronger than about how much unknown you are comfortable with.

The mechanical difference

Tirzepatide is a dual agonist: GLP-1 and GIP.

Retatrutide is a triple agonist: GLP-1, GIP and glucagon.

The glucagon receptor is the addition, and it is not a small one. Where GLP-1 and GIP act largely on appetite, insulin response and gastric emptying, glucagon receptor agonism brings energy expenditure into the picture, the body spending more rather than only taking in less.

For where both sit against semaglutide, see semaglutide vs tirzepatide vs retatrutide.

What that changes in practice

Retatrutide’s trial data has attracted attention for the magnitude of the results reported. Tirzepatide’s is more extensive and has been examined for longer.

That is the honest trade. Retatrutide is the newer and more aggressive mechanism with a shorter track record; tirzepatide is the better-characterised option with a large body of evidence behind it.

Side effects follow the mechanism

Both share the GLP-1 pattern: nausea, reduced appetite, altered digestion, and the gastrointestinal effects that come from slowed gastric emptying. These are the effects people most often stop over, and they are usually most pronounced early.

Adding glucagon agonism adds its own considerations, including effects on heart rate and on glucose handling. A more aggressive mechanism is more aggressive in both directions. That is not a caveat, it is the same property described from the other side.

Both step up gradually for a reason

Neither of these is a compound you start at the amount you intend to end at. The stepped approach exists because tolerance to the gastrointestinal effects builds over time, and skipping ahead reliably produces a bad week.

Both SerenQ pens are supplied in two strengths so the step-up does not require guessing on a single dial, but the dial still reads in units of volume, not milligrams, which is where people go wrong. How to read a peptide pen dial covers the conversion, and how many doses are in a pen covers what that means for cost.

Which to choose

Tirzepatide if you want the option with the most evidence behind it, if this is your first GLP-1, or if you would rather a known quantity.

Retatrutide if the dual-agonist route has been tried, and you accept a newer mechanism with a shorter record in exchange for the additional receptor.

Neither is a substitute for the things they work alongside. They change appetite; they do not change what you eat when you are not hungry.

Handling

Both ship as pre-filled pens, so there is no reconstitution and no in-solution clock until first use. See peptide pens vs vials. Store them properly between uses: storing peptides properly.
