Tesamorelin is a synthetic analogue of growth hormone releasing hormone. The base molecule is the 44 amino acid human GHRH sequence, modified with a trans-3-hexenoyl group attached at one end. That single structural change is the whole point of the design: unmodified GHRH is broken down in the bloodstream within minutes, and the added group slows that degradation enough to make the molecule practical to work with.
Mechanistically it acts on GHRH receptors in the anterior pituitary, prompting the gland to release its own growth hormone rather than supplying growth hormone directly. That distinction matters in the literature, because release stays under the body's existing feedback control rather than bypassing it. Published research has focused on the resulting effects on IGF-1, and on visceral adipose tissue in particular, where phase 3 work reported preferential reduction of visceral fat with limited change in total body weight.
The pen format
This is where SerenQ does things differently. Instead of a lyophilised powder that has to be reconstituted, Tesamorelin arrives as a solution already loaded into a graduated injection pen with a dial and a dose window.

Handling and storage

Tesamorelin against the other GH-pathway options
Tesamorelin is a stabilised GHRH analogue, so mechanically it sits on the same side as CJC-1295, it raises the ceiling of a growth hormone release rather than triggering one.
What separates it is the research attached to it. Tesamorelin is the GH-pathway compound with the most specific body-composition data, and that data centred on visceral adipose tissue, the fat around the organs rather than under the skin. No other compound in this category carries that.
If the interest is general GH pathway support, the Ipamorelin + CJC-1295 pen is the more common starting point and covers both halves of the mechanism. If the interest is specifically the visceral compartment, tesamorelin is the one with the evidence.
Worth being realistic: that research was conducted in a specific clinical population, not in lean trained individuals, and extrapolating between the two is where disappointment usually comes from.
