HMG vs HCG: Why FSH Is the Missing Half

HMG does the job HCG cannot: it supplies FSH, and FSH is what sperm production actually depends on.

The two hormones

The pituitary sends two signals to the testes. LH drives testosterone production. FSH drives sperm production.

Suppression from anabolic use shuts down both. Restoring one does not restore the other.

Why HCG is only half the answer

HCG mimics LH. It restarts testosterone production within the testes and preserves testicular size, which is why it is the standard tool. See HCG explained.

What it does not do is replace FSH. For someone whose concern is testosterone or testicular atrophy, that is fine. For someone whose concern is fertility, it addresses one of the two required signals.

What HMG provides

HMG, human menopausal gonadotropin, contains both FSH and LH activity. That makes it the more complete option where sperm production is the goal, and it is used in fertility medicine for exactly that reason.

When it is actually needed

Most people recovering from a cycle do not need HMG. Natural FSH generally returns once the axis restarts, and a SERM protocol raises both LH and FSH by acting on the pituitary. See clomid vs nolvadex.

HMG becomes relevant in prolonged suppression, in long-term testosterone use where the axis has been offline for years, or where a semen analysis shows a problem that has not resolved.

This is a medical area

Fertility problems deserve proper investigation. A semen analysis is inexpensive and tells you whether there is a problem at all, guessing and self-treating wastes time during a period when time matters to people trying to conceive.
